Admissions and Discharge

If you would like more information about a Health Concepts facility, contact the admissions director of the home you are interested in. They can help answer your questions and guide you in making the right decision to meet your needs.

If you choose a Health Concepts facility, the admissions director will work with you, your physician and/or hospital discharge planner to ease the transition to nursing home care. If your needs are met by a short-term stay, the discharge planner will also help with your transition back to the community by assessing what your post facility needs will be.
